I always assumed that on an United Earth, countries became states, and the entity of the USA was still one of the more powerful Earth-states, and continued to retain its basic borders and some semblence of its own history, enough for the Constiution and its wars and presidents to stay relevant. Just like some states today are bigger, more powerful, and more influential then others, in a United Earth, America is still bigger, more powerful and more influential than others.
My assumption is that "United Earth" is just a future version of the United Nations that actually works, and that countries are still sovereign.
As for Amanda, Picard all but said that Riker was a American, Kirk is from Iowa, Archer is from New York state, Sisko is from New Orleans, Sulu is from San Fransisco.
What's wrong with conjecturing where Amanda's from?
