That was from a Saturday Night Live sketch were the Enterprise was repurposed into a cruise line.

A similar exchange was part of the Carol Burnett parody from the early '90s.
 
Kirk (or someone) indicates an injured man and tells McCoy to tend to his wounds. Bones then angrily says, "I'm a doctor, not a... oh, okay," then goes to tend to the man?

It was a SNL skit, however, Kirk, Spock and McCoy had retired and gone into the restaurant business. One of the patrons was choking on his dinner.

Later Dana Carvey showed up as the new city health inspector ... Khan.

This was all during one of Shatner's appearances on SNL.
 
I remember reading about a convention many years ago, between Star Trek 3 and Star Trek 4. There was a lot of speculation about Saavik being pregnant with Spock's child. Apparently DeForest Kelley was on stage doing Qand A. Someone asked if he'd deliver Saavik's baby. Someone shouted out from the audience, "He's a doctor, not an obstatrician!"

I read that and thought, isn't that kinda the same thing?
 
I'm a doctor, not a happy, well-adjusted person!
