It's not a superhero name -- it's the name of every assassin who came out of the Red Room, from Dottie in Agent Carter to Natasha to Yelena and all the others we saw in the movie. And she's not a Romanoff (or more properly Romanova), she's a Belova. They aren't biological sisters -- it was a childhood cover identity that they chose to embrace later in life.

Given that Yelena has not attempted to Americanize her accent as Natasha did, I don't think she'd Americanize her surname either, so it would be Romanova if she did adopt it. But Yelena Belova is the name she's always gone by in the comics, so I assume they'll stick with that in the MCU.
As far as we know. Though Natasha's full name is actually Natalia Alianovna Romanova (Natasha is a diminutive of Natalia). Yelena's patronymic has not been revealed either in comics or in the MCU.
Well it was the name both Zola and the Red Skull used, and the latter even knew her father's name, which is more information that Natasha had, so it seems to be a safe assumption.
