March 17, 1989, of brain cancer and AIDS. He appeared in the first-season TNG episode "Symbiosis" (aired in April 1988) and didn't look at all well by that point.

HIV-related symptoms turn up in lots of different places and guises. If your immune levels are down, I guess whatever happens to invade first wins...

That's what I always thought - the writers wanted it to be Beverly, but that would confuse the marginal and non-hard core fans that saw the movie, so they used an actress that looked like Gates McFadden, kind of as a message to the hard core fans that "yea, it should be Beverly, but we can't actually make it Beverly".
