Agreed. In TOS, we saw a few times where someone flinched, but that was usually due to them being surprised that the hypo was administered. Not all serums are tame; I'm sure a few probably sting on site. they may, though it depends on how the item is delivered. in TNG era, it seems the substance is delivered through the skin into the bloodstream, bypassing any nerves.
