At the end of STiD. I shed a tear for the free pass I wasted.

I'm glad that Lal and "The Offspring" get mentioned, but I think Haftel plays a good role there. For a chunk of the episode he's at odds with Data and the crew, insistent that Lal be removed in your classical overreaching bureaucratic kind of way. But when Haftel and Data join forces to try and save Lal, and then Haftel reports to the crew trying to explain Data's almost emotional efforts to try and save his daughter, THAT got to me. A nice twist on the overbearing Starfleet authority figure.
I can't recall all the moments, but most of them have been mentioned.
Tapestry - TNG 6x15 When Picard gets stabbed, but it was a happy crying, because I was so happy Q gave the chance to Picard to re-live a defining moment of his life. How many times would you just love to go back and re-live parts of your life, and you can't. Well to be able to experience that, and without changing the timeline. ... Man, that's a unique gift.

@cyke, good mention of the Haftel moment a the end, it was that that got me too.
I will throw in the last scene of the perfect mate, when Picard escorts her to the transporter pad, I don't know why, but it gets me.
I will agree with everyone on "it is linear" from The emissary, and the entire episode of the visitor, I cry the whole way through.

One I haven't seen mentioned yet: McCoy (with Sybok's help) reliving his euthanization of his father in TFF.
The scene in "Duet" where Marritza breaks down is pretty powerful.
The moment that got to me the most was when The Undiscovered Country went to end credits and each of the stars "signed their work". Gets me every time!

The ending of the episode where Bashir tries to find a cure for that artificial disease created by the Dominion was pretty intense.
He can't find a cure for the people, but he manages to cure the newborns. That whole sequence where the mother dies after giving birth and he gives the newborn to the guy who was previously euthanizing everyone to spare them pain. Not only was that a fantastic twist to the typical "hero saves the day" story, it was also excellently done.
And I love that when he gets back to the station, he keeps trying to find a cure.
Not really a tear-jerker, but still powerful. In "Hippocratic Oath", the last thing Goran'Agar (the mutant Jem'Hadar who doesn't need white) says to O'Brien before going off to kill the rest of his own men:
"You are a soldier? ... Then you explain."

1. The death of Kai Opaka. The emotion Nana Visitor conveys, the anguish on that prison moon. It was the episode that made me believe DS9 could be great.
2. At the end of the Voyager episode "Year in Hell," Janeway reaches out to embrace Tuvok saying good-bye. For just a *moment* Tuvok breaks and embraces her back. That moment between them...get me every time!
3. Relics - a love letter to James Doohan.
4. The Picard scenes in the TNG episode "Family." You see the human pain behind the assimilation. A great Patrick Stewart episode.

Admiral Forrest's death in ENT.
Not only is Vaughn Armstrong my favorite actor of all time, but Forrest always reminded me of my father. When Forrest died, it was almost like if my dad had died. (Fortunately my dad is still very much alive, but watching Forrest die was almost like the "real thing", as it were.)
And you know, the MU Forrest's death hit me the same way. Because in both universes, Forrest was basically an honorable sort; both versions died while saving others' lives. Is that cool, or what?

In contrast, watch the end of Voyager: Unimatrix Zero. Janeway in sickbay, joking around after having been assimilated and had her body raped with implants. "Oh, the doctor still has to remove the stuff from my spinal column, and it's very painful, hahaha."
The Picard scene in "Family" was good (and Spock's death scene in TWOK would be right at the top of the list if it hadn't been nerfed by his resurrection in the next film), but it's this:
... that's the undisputed champ for me.
"I am alive. I will always be alive! It's Marritza who's dead! Marritza, who was good for nothing but cowering under his bunk and weeping like a woman. Who every night covered his ears because he couldn't bear to hear the screaming... for mercy... of the Bajorans..."
Still puts a lump in my throat even now. I think it's probably the single strongest dramatic moment I ever saw in a Trek show.

This. Right here. That was a heartbreaking scene and the sheer emotion really got to me.
to be fair though, she was never mind raped by the Borg like Picard was.
