How was crusher killed?

Wouldn't it be ironic if he'd been crushed?

I always found it mildly amusing that TNG's doctor was called Crusher, and when DS9 premiered its doctor appears to be called Basher. Such a violent profession.

And there's a slight discrepancy between Reunion and canon, because in a later episode, there's a reference to Jack dying on an away mission, not an EVA. But the description is ambiguous enough that it's easily glossed over.

Hey, it's hard to detach a nacelle with a phaser rifle while in an EVA suit during an away mission when the natives turn violently against visitors.
