So, the Romans from B&C and Miri's planet and the Iotian gangsters and Landru's planet etc. were transplanted Humans by the same Preservers who seeded the galaxy millenia ago. Long enough for the various race to develop differently etc. That would make them pretty old. And how is organ arrangement explained and colored blood etc.

Well in this variety, the Preservers might well engage in genetic tinkering with the human race to make it different or better adapted to their environment. In the Vulcan's case, their ability to have children with humans might mean their blood is actually just cosmetically different but possessed of something poisonous to the salt monster.
In the Trek universe, in the distant past, The Progenitors spread DNA around primitive ecosystems. Relatively recently the Preservers then transplanted various cultures around the galaxy.
Without these we would have a pretty empty Trek universe. Even allowing for the statistical liklihood that there have been or will be many intelligent races out there, the vastness of 'deep time' makes it unlikely that any would co-exist in the same time period...

And in between those extremes (though closer to the latter), the Arretians colonized worlds throughout the galaxy (and probably terraformed a bunch as well, which could be why there are so many inhabited planets around big, bright stars too young to have naturally occurring intelligent life, like Rigel and Deneb, not to mention all those supernova stars like Minara, Beta Niobe, Sahndara, etc.).
