I think the particles in a transporter stream are quantum encoded to only reintegrate eith the correct neighboring particles, hence no machinery needed for the rematerialization sequence on a planet's surface or a site to site transport.

I think an external dish is a classic and asthetically pleasing design element. Also good for dramatic effect to see the deflector dish go flying off into space, then you really know you're fuc*ed.

I've seen several 3D meshes of TOS Enterprise around the Net that don't feature the undercut (they obviously only looked at a 3-view), and the whole design just suddenly looks cheap to me somehow.
