Or then "Commander, Starfleet" is different from "the CinC". It could be argued that the former is the top man of the entire organization, while Starfleet has a number of CinCs just like the USN does - say, one per each theater of operations (cf. the old CINCLANT vs. CINCPAC division). "The" CinC in ST would then be the CinC of the "Home Fleet" area of responsibility, or the Klingon front commander, or somesuch, and everybody would understand that this specific CinC would be in charge of this particular business.
Cartwright does act a bit subordinate to this CinC guy, but there's nothing truly explicit about their hierarchial relationship. Perhaps the CinC guy sits in the meetings and Cartwright does not because the latter is too highly positioned to bother?
