Why? Shatner has aged in a certain way. But Captain Kirk has not aged. When we last saw him, he was sixty, and still hanging on to a look that was more youthful than the "Deadly Years" appearance, let alone Shatner's current looks.
It's quite plausible that, had Kirk lived past sixty, he'd have started to look like his "Deadly Years" self (which was diagnosed as corresponding to an age of 60 to 72 years) rather than like old Shatner. Indeed, the pseudobiology of the case should argue for a difference. Shatner's current face and body are due to a certain kind of diet combined with a certain exercise regime; the "Deadly Years" Kirk supposedly ate nothing while growing 40 years older.
Of course, the pseudobiology should also take into account that Kirk's hair didn't grow an inch during those 40 years - so it shouldn't have turned grey, either. Now that's something that should be redone in this episode, and in TNG "Unnatural Selection". (But not in DS9 "Distant Voices" since Bashir's aging there was all mental and not biological.)

Joking in that I can't see it ever happening, but from an artistic stand point, art exists for its own sake. Little tweaks like this would be an exercise in 'why not?' and an interesting compare and contrast to the original.
As for Doohan being dead, I haven't seen the episode for a while and I'd forgotten he aged. I've pondered this question and others for a long while, just hadn't got round to registering on the forum to ask.

But. Why not? Whats the point of not doing something, when the alternative is doing it? It hurts nothing and it could be fun, and if the result is rubbish, then atleast we've learnt something for certain that could only be guessed before.
Should Shatner re-film his scenes for The Deadly Years? The makeup looks nothing like his aged appearance.
Its technologically feasable. They should do it.
The only thing I didn't like about this episode? The blond-woman, kirk's love interest from the past. I didn't care for her at all. She gave the word "frigid" a whole new meaning...
