What level are they at? Medieval? The Apex species has spaceflight but no warp drive. SOT,but is this the first instance of hearing the original and translated language simultaneously?

I'm not sure about that. It's possible Georgiou was referring specifically to the Kelpians. For all we know, Kaminar (or the "ranches" where the Kelpians lived, specifically) is sufficiently isolated from the Ba'ul population centers for Starfleet to consider the species distinct for PD reasons. Though, I suppose violating an independent planet's territory to visit Saru in the first place would also be on shaky grounds, PD-wise, even with the distress call.
Third, in recent memory. It happened in Beyond, and in "Into the Forest I Go," when Burnham was stalling Kol on the bridge of the Sarcophagus ship (which I remember specifically for establishing the UT is, apparently, psychic or pre-cognitive; I don't speak Klingon, but I could sure tell the translation mentioned "humans" at the beginning of a sentence, while Michael said it at the end).
And then there's ST Beyond, where the UT initially lets the native voice of the traitorous "survivor" be heard alongside the translation. Later on, she "learns" to speak plain English, supposedly thanks to the UT finally learning her language and starting to do its usual thing. In DS9 "Sanctuary", the transition is different, with individual words in alien speech gradually replaced by English counterparts, but all of it coming out in the single voice of the speaker him- or herself.

Although it's fairly seldom that the phrases actually overlap.)
Debate apparently rages on whether the Ba'ul are the Predators that are implied to be native to Kaminar and the reason the Kelpians are so wary of the world. The Ba'ul could be extraterrestrial saviors instead, culling the Kelpians as payment for keeping them safe from the Predators.

Though you just reminded me of one of my favorite examples, in "Future Tense," where we hear the Tholian's shrill chittering, and, bonus points, the translated voice is flat, '90s-style synthesized speech, while the UT usually provides inflection. I liked the implication that the Tholians were alien enough that the UT wasn't even going to try and guess at anything beyond the literal meaning of the words they were saying.
