This folio (or one like it) was repurposed as Ambassador Fox's er.. well, some kind of official looking document cover, in A Taste of Armageddon. Someone printed his name on the cover.![]()
The document calls Spock "Half-Vulcan Science Officer Spock" but doesn't call Pike "Fully Human Captain Christopher Pike." Double standard.
It also says "no Human should ever visit it again." Does that mean that Spock and Pike would support a non-Starfleet vessel with no Humans aboard going there?

Not quite true. We don't see the Grissom's screen, so we don't know what it might or might not have shown. The refit E had them in the first two movies. In TMP, you can see them when Kirk orders a tactical display on screen of their approach to V'Ger at the 0:58:36 mark, In "Khan", you can see them as Kirk comes onto the Bridge just before Enterprise launches at the 0:29:50 mark, though oddly they don't show up in other Enterprise or Reliant viewscreen shots.
