Yep. From against the pubic bone (lightly) to tip for length, and from the widest part all the way around for girth.
Um... *Some* people exaggerate, mostly in a joking way though... When I've actually had legit level conversations with guys there's rarely THAT level of exaggeration... In fact, of the guys I've had enough experience with, they were all being fairly close to accurate... An inch and a half is a HUGE difference, a bit much to use as a specific number to chop off every guy's claim of size...

Usually the widest part is near the middle, but it definitely has to be at the widest part, otherwise the circumference is going to be wrong. It would be like taking measurements of someone's waist in the middle of their body, instead of where it's actually located.
But isn't it wider at the base of the head?

In general. I use the term "near the middle", but yeah, it's often at the base of the head, but again, every guy is a little different.

Interestingly enough, there are incidents where the head is smaller than the shaft, or the base of the shaft is wider than the base at the head. I'm not 100% on any of this, because it differs for each person. "In the middle" is a really vague term, so I'll say right off that I may be wrong there. I do know of some where the base of the head is narrower than the middle of the shaft. I'm not sure of the majority though, and I wonder if that question has been answered somewhere.

This doctor was quite the culture buff and there was NO gossip magazines just stodgy journals about architecture or opera. But I did find a medical journal. I opened it up at random and OMG!!! It was a full double page spread of photos of diseased penises.
There was at least 60 photos. They were numbered so you could refer to another part of the journal and see what those lesions and weepy bits were in medical terms. It was quite a shock.
Anyway that's the most I've ever seen in one place and I didn't take too much note of the size of the heads in relation to the shafts.

No, that's cool. I suppose it makes sense, in that a vagina wouldn't care overmuch where the circumference is coming from, so long as it's there (unless there's something I'm not considering).

I'm wary of porn videos, if only because trick lighting and angles are used and so that distorts a clear image. From the various non-porn, clearly visible times I've seen them, they're all different and vary greatly in some cases.

The general accepted technique I read says from pubic bone (which pushes into the little fat cushion a bit) to the tip, and to repeat the measurement several times to get an average. This is because the penis actually extends to the pelvic bone as the usable part of the appendage for sexual interaction. At least, that's what I think.

A sewing tape measure and a little bit of effort will yield more accurate figures, but I figure most guys just guesstimate anyway.

I used an actual metal tape measure once. Two words: Ouch.
It's not difficult -- just hold a ruler up to the thing. Are most guys really just wildly guessing?
To be more exact, a sewing tape or even a piece of string would be better. A rigid ruler really can't measure a wang accurately if you have a curve.
