Worst DVD ever made?

Any release using 2-sided discs. Thankfully there aren't many of those around today, the only 2-sided release I own is BBC's Yes Minister/Prime Minister collection, which inexplicably consists of four DVD-9 discs, two DVD-5 discs, and a single DVD-10 disc.

The R4 Farscape box sets are pretty crappy - no subtitles, pathetic extras, and I have two or three unplayable disks in my collection. Very ordinary.
 
Any DVDs where they just can't be arsed to put anything on the topside of the disc. They just end up getting covered in finger prints on both sides.

The Friends ones are a prime example. They're also double-sided, just to make them even worse.

The first set of the Farscape Starburst backs weren't much different in terms of quality, they were double sided discs and worked like crap.

Another issue, the first release of SG1 season 8 only had the edited down version of Threads.

I had to wait for them to fix both the Farscape and SG1 dvds before I finally bought them.

You do realize that neither side of a double-sided disc can be printed on (except for the inner edge)?
