I always had the impression from the dialog in "What Little Girls Are Made Of" that Chapel was a researcher who signed up for Starfleet as a nurse in hopes of finding the missing Roger Korby. Perhaps with that background, she had already completed must of her doctorial program and was just shy of an medical degree. After the FYM, she just returned and finished it or earned the remaining credits while serving aboard the Enterprise.

According to the novels, he resigned as Ambassador after the new President was elected and turned down being reappointed, returning to Starfleet and was offered the Titan XO job over Riker's head but after Data's death in NEM, he remained on the E-E and is now First Officer.
