Another vote for "A Private Little War," but I would also add to the ballot "Charlie X" and "Who Mourns for Adonais?" - two episodes that ended in much the same way, actually. (And each was the second episode of its season to be aired.)
 
"And The Children Shall Lead" has the saddest ending because the death of Gorgan meant that Gorgan couldn't have a part in the spinoff series Assignment: Earth as Gary Seven's wacky elderly neighbor. Very sad.

Okay, it's really "City." Or "Conscience Of The King." I was only joking about "And The Children Shall Lead."
 
I'd say "City...", but there's a lot of other candidates.

I always thought the end of "Requiem for Methusula" was kinda sad...

I never did. They never sold me on the idea that Kirk was that ga ga over Rayna that he should have had a meltdown over losing her, certainly not to the extent that Spock would have to wipe his memory just to get him functioning again.

His reaction to losing Edith was much better sold.

That said, I always found the ending of "The Way to Eden" to be particularly sad. Severin's followers didn't deserve what happened to them in the end.

Would have to watch again to be sure, but I think CHARLIE is the first time we hear the 'sad' music that plays over the end of PRIVATE, after Kirk says, "We're very tired, Mr. Spock - Beam us up home." So once again, like the action cues in DOOMSDAY, music makes a huge emotional impact on the final product in TOS.
 
Another vote for Charlie X.

One that no one has mentioned is "Let That Be Your Last Battlefield", although the ending could better be described as "tragic" rather than "sad".
 
I ca't say it qualifies as the most sad ending, but let's not underestimate the tragic ending of "The Alternative Factor."
 
"A Private Little War" could also be a candidate for this poll.

I thought of that episode straight away. It ended so suddenly, but that was what made it so grim. Even Kirk seemed to be giving up on the situation as hopeless.
 
"The Alternative Factor" to be trapped in a timeless void with a madman at your throat until time itself ends. Close enough
