What about the EMH Mark-1 (Andy Dick)? Or Dr. Sellar (Suzie Plaksn Vulcan)?

Really though, I'm not too picky; as long as I can tell which end of the CMO I'm looking at, I'm okay. Sorry Phlox.

Dick was EMH Mark 2. Picardo was Mark 1. Selar and the EMH's were not generally Chief Medical Officer (except for the Voyager EMH), and I believe Trelane's lists are meant for the hero ships.

In order of appearance, not preference.

Who I didn't choose and why: Bashir is a little too smug and holier-than-thou, The Doctor can get overbearing, and Phlox is okay but a little too bland. I like Boyce, but we don't actually see him do anything as a Doctor. Thus I can't include him.

Who I did choose and why: I like McCoy and Pulaski because they have very forceful personalities. I like Crusher because she's very understated but also very passionate at the same time.

Before the third season of DSC, I wouldn't have listed Culber, but during the third season Culber really came into his own. He's a great Doctor and a great Counselor too. How he handles Georgiou in S3 is a loooong way from how he handled (or rather didn't handle) AshVoq in Season 1.

EDITED TO ADD: TOS McCoy and Kelvin McCoy are pretty similar. There's not much of a difference, if any difference at all. So I think including both of them in my list feels too much like cheating. TOS Kirk and Kelvin Kirk, I can see a difference. TOS Spock and Kelvin Spock, I can also see a difference. TOS McCoy and Kelvin McCoy? Not so much.
 
And, for no reason, here’s my thoughts which have little basis in logic….

McCoy - he’s classic. Didn’t like his character much in the movies, but in TOS he was a vital part of the show.
Pulaski- I had lunch with the actress and she’s a delight. We drank tequila and had some laughs
The rest….whatever
The last… crusher - I had lunch with the actress and, perhaps I caught her on a bad day, but she was less than delightful. Or perhaps it was me, but either way it was not an enjoyable time.
