What I remember from this episode is a scene where Kes offers to give a lung to Neelix, and Neelix says something like, "No! Let someone else do it, not you!"

That still bugs me. It's like he didn't much care about anyone else on the crew losing a lung, and it assumes at least one other person would volunteer. Yeah, that's the woman he loves, but it just struck me as incredibly callous.

And why didn't they take one of Neelix's own lungs back from the Vidian who had it?

Giving up an organ isn't like trading baseball cards.
Because he does care for her, he doesn't want her to give up a organ for him. It's not callous, he's considering her well being before him own. He's saying he'd rather die than have the women he loves be operated on for him.

And why didn't they take one of Neelix's own lungs back from the Vidian who had it?
They said very clearly in the ep. that they couldn't find it because it was already in a new body. You can't just give back an organ once it's in a new body, it's causes shock and damage to the organ thus making it good for nobody.
