Wasn't Sisko at fault for the outcome of Change of Heart?

If memory serves, Jadzia and Worf weren’t typically assigned on missions together once the war started. He was usually on the Rotaran while she was on the Defiant. And when he was on the Defiant she was on the station.
In this instance Sisko is at fault in a sense because he is the CO but he also didn’t assign them to the mission, Kira did. Plus that mission wasn’t supposed to be a rescue/extraction, it was a spur of the moment thing. Worf still let personal feeling doom the mission which is why he got the brunt of it.
although I’m sure Sisko got chewed out a bit from Ross or whomever.

Kira didn't know Worf and Jadzia would need to land and have a long, dangerous hike to extract Lasaran when she assigned them together. Their assignment at first was just go to designated coordinates and receive his transmission.

They were never on a mission together during the time they were married, except in that instance.
They shouldn't have been assigned together by Kira in the first place, but to be fair, they were the highest ranking Starfeet officers on the station at the time, and time was a factor. Given the highly secretive nature of Lasaran, it's very likely only officers with a certain security clearance level high enough could be assigned. Both are Lt. Cmdrs., which is high enough, I'd say, for a LOT of things.
I don't think Sisko can be faulted here, even incidentally. And he DID give new orders that they were never to be assigned together again.

Yeah, even acknowledging that the mission parameters shifted, that they went on the basis of intercepting a transmission that their contact demanded they make it into a rescue mission... It was a mistake to have a married couple take this mission alone, and I feel like the fact that Kira is the one handing them this assignment, meaning someone not in the Starfleet chain of command, is the script acknowledging this and sidestepping the obvious question of the suitability of these two for this sort of mission as a result.
Like when Sisko says at the end they weren’t going to be assigned together again... Shouldn’t that have been the policy from the start of their relationship? Isn’t that standard throughout hierarchies across careers, not even careers with military-style chains of command?
I love Worf and Jadzia, as characters and as a pairing, and the plight that Jadzia goes through and the struggle of Worf between his duty and his emotions is heart wrenching to me. But the script fails to address this very noticeable elephant in the room, and that has always brought down the episode as a whole for me.
