Depends on the image and underlying problems. In Photoshop, with 3rd party tools like Topaz ReMask 5 to make masking less of a chore, simpler edits take 2~15 minutes to do. I tried something bigger with Sulu's head and Sheldon, but after an hour I decided to swear and use cheats because I wasn't able (or was too lazy) to find a similar camera angle of the same area to get at the bridge set to put in place of where I wanted sheldon's head to pop out but that bit of set is in the background that isn't easy to manually recreate with Photoshop's tools. The text bubble helped hide big problems I didn't have the time to blend in... but copying and pasting existing scaffold, rotating, flipping, using dodge, burn, and smudge, etc, to blend took forever and it still didn't look sufficient for use (regardless if I overlooked a few mistakes or not).

Even then, such as Tuvok in front of his quarters' window facing outer space, the masking tool isn't perfect with a ton of fringing so I have to manually go in and erase all of it carefully. And then I look for darker backgrounds if I can't remove the fringing fully (ReMask is good but on darker colors the fringing becomes a problem as it can't tell where one object ends and the other begins), since lighter backgrounds would reveal the fringing. Tightening up on the mask controls to refine the edging also helps save time. Or I'll take the lowest layer and do a manual lasso select with slight feathering to get around fringing (e.g. Tuvok's hair). There was once an image of Riker, Geordi, and Data on the briefing room where I put in the Doctor's TARDIS and galaxy (7th Doctor opening credits). I didn't realize I overdid the masking and removed Riker's shoulder in the process and of the three characters, he was on the right, but not so close to the edge that he'd be overlooked. Yet that's the one I screwed up on. Fringing on all the briefing room windows was a HUGE chore so I gave up on trying to use the roller coaster POV shot... the Dr Who credits were far better anyway, even without Riker's shoulder...

I should download the HD version of that scene once I find the episode and redo it with screencaps from a McCoy story (in PAL, 625 line for the extra vertical resoluition) and then use the remade image as my wallpaper... on the image above, I remember spending almost 5 minutes carefully removing haloing from around Data. It's not perfect but comes close...
