I used the "TOS Starfleet Officer Duty Uniform Female" from Roddenberry.com. It's a finicky pattern and I made a lot of changes, but it was a good base.

I wish I had the sewing ability to make one... I desperately wanted to have one for Halloween this year, but I can't sew worth beans, and don't have money for a professional tailor. Kudos to you for having the patience to make this fantastic uniform!

Thank you! Though, would you like to know my dark secret for the flattering fit? I had some help, in the form of shapewear...there's quite a lot going on under the dress to help me look like that.
Honestly, despite some of the headaches this uniform caused, if you can sew a straight line, you can make this dress. The rest is just trial and error (sometimes lots of error!). Give it a try--use cheap fabric to make a mock up and work out the fit issues, probably mutter and curse a bit along the way, and you should be good to go.
