It is interesting to note - though not definative - that the Mirror Earth Starfleet does have the rank of Lieutenant Commander, suggesting that Earth Starfleet also has it and we just didn't meet any through the series.

As part of my rewatch I just started the fourth season. When T'Pol returns from Vulcan she's now Commander T'Pol of Starfleet. With Commander's rank pins on her old outfit. Which I don't mind. With her around you can always tell if it's too cold.
