In that episode, the cadets were given field commissions by the surviving senior officer, who was the most senior cadet/midshipman. As per military parlance, the next in the chain assumes command and if necessary can promote from within, which is what "Captain Extreme" did when the Valient's actual captain perished. While a good entry in DS9, a better example of this happening, fiction-wise, can be read in David Feintuch's military SF novel Midshipman's Hope (where I get my board name from).
