I'll go with the EMH for one reason: He has the collective knowledge of all those doctors in his matrix.

I wonder if you could alter his appearance to be one of the other Doctors?

Hey!, when was Bones ever mean? Granted he was all the other thing you said but he was never mean.
Heck on occasion he could be tough, there's a scene in "Space Seed" when Khan found out how tough McCoy was, McCoy even gave directions on how best to cut his(McCoy's)throat!

Hey! On TNG there was a hot Vulcan Doctor, think Selar was her name, well that's two for TNG, Bones may not have looked anything like Crusher or Selar, he's still my pick!

Crusher - I like her bedside manner and knowledge. Seeing more than one doctor I can tell you that being knowledgeable and having a good bedside manner trump most everything else. She seems more at ease with her patients.
The Doctor - Has instant access to the entire Starfleet Medical library.
Bashir - Close to the Doctor in knowledge and skill.
McCoy - Good doctor, but 100 years behind the others.
Phlox - Also a good doctor, and uses a lot of what I guess would be called holistic medicine or alternative, natural remedies. He would get a little getting used to, but even now doctors are re-evaluating the use of leeches.

Dr. McCoy--Because he's a doctor, and not much else.

Dr. Bashir--Second only to McCoy in my book.

Phlox is my runnerup to Bones McCoy because he reminds me of one of my favorite Uncle's! He was into herbal medicine.
