To me, the most offensive thing you can do to Kirk is steal his ship. And who did it? His friend and first officer, which cuts twice as deep. So, I have to guess it is ...

Oh ... I get it. The next clue is not seen, as if it is obscured by a cloaking device.

I will guess "Balance of Terror" .

1. Kirk could have been offended by Stile's brazenness.
2. Invisible clue means cloaked and the Romulan ship is cloaked in this episode.

If that is not correct, I think I will get it on my second guess.
 
Ah, "Court Martial".

1. Kirk could have been offended by many things in this episode. Stone and those two guys in the bar were particularly insulting.

It seems the second clue was just playing dead, like Finney.

2. The sound detector for the heart beats was infallible.

1. Another episode where Kirk could be offended by many things, although in a less serious context.

2. Tribbles are infalible "Klingon detectors"
