Hmm I actually choose this episode as my guess because: "Only five" Enterprise members beamed down but only "four left" the planet after the Yeoman is killed.

That works too. Poor Yeoman Thompson. Her death and the lack of justice for it bothers, even occasionally enrages me like just about nothing else in TOS, although somewhat ironically I count By Any Other Name among my top 20-25 favorite episodes.
