Most likely systems to fail in any series or movie

From my viewings of the series and movies it seems the most likely systems to fail on a starship are

1. warp core ejection system.

2. auto destruct.

3. Transporters.

Whenever you want these systems to work they never seem to.
 
You missed the most famous one, which was even used as a joke in DS9. The Holodecks. I am shocked the amount of times people would still use the Holodecks after they kept failing.
 
100% right.

The holodecks are pretty much death traps. Darn it to heck for all the times they mess up and for whatever reason you can't just pull the plug and turn them off!
 
The 1701-D's starboard power coupling probably failed at least 3 times per season!
 
^ Funny you should mention that. I remember a conversation in a sci fi store about the time the Defiant was introduced to DS9. We'd all noticed that the starboard power couplings were mentioned as going during the Jem'Hadar attack.
 
Don't forget the artificial gravity which was very frequently down to "point eight"...

It's good to know that some people just never, ever learn. Even in the future.
 
Shuttle craft almost always fail to touch down on planets smoothly. Usually they end up wrecked. Unless it's visiting a planet to rescue a crew member already trapped because of a transporter failure, in which case the rescue is flawless.

And the turbo lifts. Don't forget how those things can break down, or throw people onto the bridge at warp speed, or open up into space, or take you to the wrong deck "this isn't engineering", or get caught up in temporal anomalies and you see yourself when the door opens. They're especially unreliable if the travellers don't like each other: Odo and Lwaxana, or Picard with children.
