Hey, if they'd actually built the super-sized Enterprise of STXI, maybe they'd have been able to squeeze Khan's fake chest inside. Stranger things have happened.
Even Mr. Montalban swore it was actually HIS chest...
People lie. Does anyone believe he didn't wear a wig (as Khan *and* in real life)?
Ignoring the fact that this image is reversed, I'd like someone to explain why his sternum is so wide, or what those black lines beneath his pecs are all about:

It's either the worst make-up job in the world, or there's something more than make-up going on. That necklace and shoulder strap are probably in service of something more than simple costuming, too.
Further, Ricardo Montalban said he got into shape by doing "lots of push-ups". It's almost a non-answer. No-one gets their chest into that condition without serious gym work, least of all a man in their sixties.
Call me a cynic if you wish (I prefer the term realist), but I remain unconvinced.
The thing that makes it look odd in the film is that the top he wears has some support effects, so it makes his real chest look a bit too high, like a modern boob job. It is his real chest alright... look at the terrible quality of makeup effects in the film, and how bad they are photographed. No way that Keppler and Rescher could make a fake appliance look good on film, no way at all.
And Montalban was showing less well-preserved skin than similar-aged-but-in-shape Kirk Douglas did in SATURN III a couple years earlier (and no, please don't ask for screencaps, they will sear your eyes.)
EDIT ADDon: the line you mention in the pic is a bad shadow on the NATURAL sag-line of a guy breast or a small natural chick boob. Check out PLAYBOY issues from the 70s (not centerfolds, more like the 'girls of' parts) and you'll see plenty of natural small boobs that look this way.
