Both were students of actor Jeff Corey, so perhaps they were friends, and this is a candid shot, not a still or publicity photo.

She's clearly giving him the mind meld, and his eyebrows are partially shaved, so it clearly during Trek.

You're right. He does appear (to me) to be older than his 37/38 years when TOS ended, but his face was fairly lined even then.

But I'm still wondering if it was just a "goof off" shot of them taken in another setting...

A mystery to me...

Age-wise it looks like Nimoy during Star Trek III.
