At least Fios is rapidly vanishing. Episode 1 was missing when I started watching. After the second day, episode 2 vanished too. So you may want to check if the program still available.

I missed an episode, too. On Demand had episode 1, but none of the others. I contacted my cable company and BBCA and got episode 3 about a week later. It's hit or miss with On Demand. It's like episodes are made available if a flaky person remembers and bothers to do so.
It could be some part of a licensing agreement where the original broadcaster has the opportunity to re-run before any other outlet can run it.

FWIW, it appears that ITV's website allows you to rent episodes for a pound thirty each, so it's possible to catch it that way. You can also buy them on Amazon for about the same price (2 dollars American), but you have to wait until BBC America airs it first.
On Demand depends more on your local cable provider and who provides them with their pay-per-view and on demand service. Episodes of some shows can be lost, delayed, or even corrupted if they're sent in a huge batch of episodes from other shows, movies, and specials. Calling your local cable provider works, but more often than not they have to call their PPV/VOD provider.
