Believe it or not, Shatner pressed for their to be a fistfight between Kirk and Khan in the film and, to this day, seems to think the movie would have been better with it. He finally got his wish, at least, in TSFS with Kruge. Personally, I think it's kind of unique and good for the film that the protagonist and the antagonist never actually meet.
For the life of me, I cannot figure out how or where to put a physical fight between Kirk and Khan in the film, unless Kirk's taunt of Khan were to prompt Khan to beam down to the Genesis Cave to take the Genesis device personally. Same reason he does in the actual film, to leave Kirk to rot, knowing he's lost, buried alive...If Khan beams down in force, confronts Kirk and party, and physically takes the Genesis device, why would Khan leave them alive? I can't think of a good reason for letting Kirk live in this scenario.
