Because they don't always release episodes in the order they were filmed. WNMHGB was the third episode broadcast, even though it was a pilot. The Empath was filmed before the Day of the Dove but shown six weeks after it . I once saw an episode of Wild Wild West where Jim West jumped through a plate glass window and had a different hairstyle when he landed outside.

Yeah I knew that too but considering the change in Scott's appearance I thought they would have changed the order that's all!

That's probably a low priority when they're trying to get an episode in the can and on the air.
Well when they're filming the episode, the order that it goes out in is not their concern to be honest!

The greased DA look and then his flat grey top! It was such a change in the look of the character that you expected it too have taken place over a couple of years! Yet the broadcast order had Scotty going back and forth, Black to grey and back again! Maybe he was wearing a 23rd century version of Grecian 2000?

Yup. Don't want to damage the permanent set piece.
More likely it was cheaper and faster to add that on than to cut a hole in the actual setpiece.

Ahh....much like the added consoles on the TNG bridge in GEN. Added just to have them explode like a giant powder keg had been stored behind them!
^ Or the added bits on the sides of the Enterprise-B, also in GEN... just there so the nexus can damage them, without needing to mess with the base Excelsior filming model...

Holy crap - I've probably seen this episode 50+ times over the years, but I NEVER noticed that at all before.And a slightly different view:
So now we must speculate on the in-universe explanation for this extraneous piece of equipment that was apparently installed during the course of the story.
