Glaring Omission From "Threshold"

There's only one answer to your questions:

The events in "Threshold" never happened!

It was just a nightmare Paris had after eating too much Leola Root stew or drinking too much of Neelix’s coffee.

The best evidence for this episode being nothing but a nightmare is the fact that no one ever mentioned the events in the episode in later episodes.
No, I think it was Rhuludian crystals.

According to the book "Pathways" written by Jeri Taylor, Neelix was using Rhuludian crystals after the tragic events when his family was killed. He and Wixiban used those crystals for a while ...

So the next dinner, he poured his remaining stash of Rhuludian crystals into Tom's food.

The result was Tom's hallucinations which are clearly described in "Threshold".

Any ill-conceived VOY or TNG episodes are clearly more of Barclay's holodeck programs. We already know he writes things like that. Just like any problem ENT episodes are clearly Riker's programs.

And I'm certain that any problem TOS are Bashir/O'Brien's holosuite programs. I can already see them writing "Spock's Brain."

It was a completely stupid episode, I mean TNG's enterprise once travelled at warp 10 and Picard did not even flinch when Geordi stated they had gone to warp 10.

Honestly I don't know I saw it on one of those continuity videos on youtube but it was probably during the first season as geordi was the pilot. If I had to guess it was probably the borg/Q one but when geordi announced they were passing warp 10 there was a complete lack of reaction on that information.

Following voyagers reaction to the impossibility of warp 10 there should have been some reaction.

Okuda's technical manuals explained how warp drive worked in TNG before season one started.

It's likely that most of the first season was written and produced before any of the writers had seen an episode.
 
I wouldn't try to logic your way through anything written by Brannon Braga.

Interesting, but they all were 'happy' via programming. He believed she wasn't staying of her own accord. but you could probe the idea that he did it for selfish reasons because he's too pansy to Captain. but I digress he was feeling that, she was programed against her will and the ship wouldn't be the same without her.
