Defensive sarcasm. He obviously isn't going to say no after all they went through to get to that point.
It was more Kirk’s reaction. I always took it to mean asking ‘where’s the bathroom?’

They had all just sacrificed their careers, and possibly freedom. Kirk, his ship and son on top of that. "Hell of a time to ask." was the perfect response to the dangers the Vulcan priestess just presented.
McCoy had already responded to the priestess. He’d committed to the process the moment they broke him out of his cell abound for the promised land.
After answering the priestess, McCoy had a private silent exchange with Kirk, and said, to Kirk, as an aside, “hell of a time to ask”, Kirk responded with a knowing “oh”, and McCoy took his place in the fal tor pan.
I took that as him needing the loo, but it being an inappropriate time to ask. Which is entirely consistent with McCoy’s dry wit.

When McCoy said "I choose the danger," I always thought Kirk had an expression of concern that kind of said, "Are you sure?"
