That was made 6 episodes after the reveal in "DOCTOR BASHIR, I PRESUME", so everyone already knew.

I don't really believe he was as stupid as it's implied he was pre-enhancements. He likely had a learning disorder making him uninterested in what was going on in school, thus explaining why he couldn't identify a cat from a house or whatever. However, had he matured without genetic modification, he could have ended up becoming a person of moderate intelligence. Maybe he wouldn't be graduating second in his class in medical school, but he wouldn't be an imbecile either. I've always suspected young Julian's parents were embarrassed by a son who wasn't as intelligent as they expected him to be and they jumped the gun and arranged for the genetic modifications to make themselves feel better more than anything else.

Kids in the 24th century seem to have much higher academic standards than 21st century kids. Weren't the 5th year (ish) kids on TNG doing calculus? I believe I remember TOS had some sort of accelerated learning tech too. So maybe Julian was just considered stupid by comparison to his peers.

I don't really see that in the episode. I don't remember what the clinical definition of "imbecile" is, but what kind of occupations would there be in the 23rd century for someone not very bright, probably reads very little or not at all or learn multistep procedures? Most of the grunt jobs would be taken by semi-intelligent machines. Even if nobody starves or has to live on the streets, people need occupations to feel valued.
