It's an early promo photo -- look at his WNMHGB uniform. The same series of promo photos includes James Doohan holding a broken... something. They seem to have just grabbed props from the studio archives for these shots.
 
Somehow I knew just from the title that the thread would be about that photo. It's memorably weird.

I notice another odd shape or two on the table at the bottom. Maybe these are meant to be alien plant/animal specimens? Sulu was originally portrayed as a scientist (well, an astrophysicist, but SFTV writers can't resist portraying scientists as experts in everything). Or maybe this was taken at a time when his botany hobby had been established as per "The Man Trap." Could these be bits of the exotic plants in the Sulu/Rand scene there?
 
Sulu was originally intended to be the ship's morale officer, who would frequently see to the crew's emotional well-being with his marionette show.

Given Sulu's keen interest in botany (see his bio), I'd venture to say it's intended to be some kind of alien plant. In reality, it could be part of a costume that was in general wardrobe and those "pipe cleaner" ends are used to attach it.

The photo is an early promo photo, circa early first season; however, it is not from the same set of WNMHGB photos.

Look at the uniform. It has the wavy rank braid and larger, more-defined insignia of the series proper, not the straight-gold band and smaller, less-defined insignia of the second pilot.

Moreover, the tunic has a jersey, raglan sleeve, which the pilot uniforms didn't have.
 
Aw, he's just fixing his feather boa for a "fabulous" night on the town!

(Come on, you knew someone was going to suggest it sooner or later! ;))

Though, a bit more seriously, I've never seen that particular publicity shot of Doohan. It's amazing how "new" material keeps turning up after 45 years!

Could it be the uniform from "Corbomite"? I bet we'd see a high dark collar if his arms weren't in the way.
