Well, I was a pretty dedicated smoker from the time I was around 18 until I quit.
I was at least a pack a day guy, and if I was drinking I might smoke two packs in a day.
My routine would usually be on my way to work I would stop at 7-11 for a coffee and a pack of smokes.
One day in November of 2005, I skipped it because I woke up a little late, and I didn't want to be late to work. I had probably five cigarettes left.
After I'd smoked those, I threw away the empty pack and went back to work.
About two days later, I realized I never stopped for more. Then I realized I had gone two days without smoking without even thinking about it. Then I realized I didn't want any then either.
So basically I randomly became "un-addicted".
I enjoy watching House Hunters International.
My mother-in-law's friend was just on that show recently. Actually my mother-in-law was in a clip or two as well!

Bears, regarding #5 on your list, I envy you. It takes a certain talent, creativity, or skill to be able to do what you do. I realize you don't have to elaborate, but do you dislike the hours or salary and benefits?
