Thomas Riker and his promotion

Until you realize they did the episode to lay the groundwork for just such a storyline, but Frakes didn't want to do it.

I recall reading comments by Frakes said he didn't really mind what they did as long as they still wanted someone "with a beard" on the bridge. I'm not sure if they were made in the Starlog TNG mags or an episode guide book.
 
They could have just killed Will Riker off, promoted Date to first officer, and made Thomas Riker the operations officer.

Then Spiner could emulate Frakes & make him smell stinky nuts!
 
I found the whole episode to be frivolous. The only thing i liked about it is that it provided some backstory on Riker and Troi's relationship before the enterprise and it kind of set up why Riker would be angry at Worf for dating Troi.

The most frivolous part of the whole thing is the TPTB completely blew a chance to promote William Riker to the captaincy he deserved, promote Data to first officer, Worf or Geordi to second officer and make Thomas start working his way back up the ladder at a bridge position. It would have solved the ridiculous situation of Will Riker turning down promotions but allowed Jonathon Frakes to stay on the show and maybe even stretch his acting abilities by playing his slightly psychotic twin.

US aircraft carriers have two captains. The Movie-era Enterprise had two or three with the rest being full Commanders.

Riker should have been Captain after BOBW, but could have stayed with Picard. Perhaps with this episode Riker could have then left for another ship, as you suggested.

I think that was the plan and then to put Thomas at Ops, but Berman decided that it was far too late in the series to reintroduce this new Riker to the cast.
