Seriously, the flask he brings with him onto the shuttle that he shares with Kirk? The thing is TINY! It could hold like two sips of alcohol! Either the inside of it is like a TARDIS, or it's the worst flask ever.
 
McCoy likes boubon and there's some pretty strong bourbon out there.

And, maybe that's just his flask for shuttle trips....his carry-on has a fifth in it!
 
^Exactly. Easy to hide and just enough to carry him until the shuttle made it to the duty free shop.

And who knows what might be in that tiny flask; could be powerful stuff.

I had a flask (among many) which was just a bit bigger then that one.

Due to the small mouth piece you only get a little bit at a time, thus it goes a long way, more then you'd expect.

That sized flask for me alone, if I nursed it like normal, would probably last me about an hour to an hour and a half with some strong stuff in it.

I guess it'd be like a glass of wine or a smoke..... just enough for a taste and perhaps a mild buzz.
 
I have a flask like that, and trust me when I say, that amount of bourbon or straight whisky is enough to calm the nerves enough to forget your in a space ship, or airplane in my case.

Injured Civil war soilders went into surgery with less whisky in their system, and still sang a battle hymn over the sound of the saw cutting off their limbs.
 
They use shuttles to get around in the atmosphere, too. In the scene on the steps of Starfleet Academy, you can see small craft traversing the background sky above Kirk and Bones.
 
Perhaps Cadet processing and evaluation takes place on a space station to ensure the recruits can handle the environment.
 
The shuttles are atmospheric craft, too. They wouldn't use the transporter for so many on a routine basis. There's really nothing else they would do.
