Frankly, I like the idea of holo-help - I find it extremely credibility straining that Voyager survived with a medical staff of TWO for the first three years, and then having the medical assistant being the SHIP'S BEST PILOT. From an in-universe perspective, I don't understand why the ship's best pilot, who would need to be at the helm during emergencies, is the 'most qualified' nurse. I mean, wouldn't Chakotay's crew have had a medic on board themselves who could take on duties in Sickbay?
I mean, what would be so wrong with pulling a couple of people and saying 'you and you are going to be working in Sickbay, brush up on your medicine,' and give the Doctor a story early on about establishing himself as the ship's CMO, not just a highly advanced lightbulb. And if they didn't do that, whip up an 'intern' hologram or two - someone who is learning from the Doctor and isn't required to know all these complicated medical techniques from the get-go. It would even help the whole holo-rights crusade Voyager launched itself on, having more holograms who develop sentience. A one-man crusade in an unusual case is hard to go on. Add a few more characters, and suddenly, he's got a species on his hands.
