Perhaps it is like the cops who jack a round into their pistols as if the normally walk around with them unable to fire. Just a dramatic effect.

No. While there are some indications that the 3 shows share a universe, all 3 series were created independently of each other. (Actually, it's pretty easy to link Friends & Mad About You together since Friends frequently brought in Ursula, Phoebe's twin sister, who had already been established in a recurring role as the ditzy waitress on Mad About You. I think there was also a Friends episode where the gang was dealing with the blackout that was started on Mad About You when Ira was trying to help Paul & Jamie steal cable. On the other hand, there was never a Friends/Seinfeld crossover. There was a Mad About You episode in which we learn that Paul used to live in what is now Kramer's apartment. However, that's generally considered negated by a later Seinfeld episode where Susan refers to Mad About You as a TV show.)

^Yeah. I always felt that was one of the big mistakes Enterprise made. It didn't have any former Benson actors in the main cast. 
Weren't the Naked Gun movies a spin off of a TV show? I think it was called Police Squad or something along those lines.

What about shows that have region specific versions made, do they count as spin offs? If so then you'd get a lot of reality shows like Big Brother or Survivor, or things like The Office UK, US, etc

And of course there's "Holby Ci+y" spun off from "Casual+y" (as they like to call it these days).
lets not forget Holby Blue, which spun off from both shows, the first series featuring Charlie from Casualty, the 2nd series featuring a full crossover with Holby City.And of course there's "Holby Ci+y" spun off from "Casual+y" (as they like to call it these days).

im also thinking BBC 3 could have a show called Holby Vice, a "cutting edge gritty urban" drama about the seedy underworld of the city of Holby, and the characters that make it up.
