The shocker here, for me, is that Chakotay remained so high throughout the series. I never would have guessed.
Tom is a big focus in season one until right after “Ex Post Facto”, which Jeri Taylor in particular felt did serious damage to the character. Between the remainder of season one and the first half of season two, he’s got very little to work with, so I’m not surprised that sets him back so much.
I reckon many may be surprised that Seven, even despite only appearing in four seasons, never climbs especially high in the rankings. But I can see it. She’s central to a great many episodes, but she doesn’t tend to say a lot as a rule. There are obvious exceptions, chief among them some of those aforementioned episodes. But, as a person, she’s just not so prone to speechmaking as most of her crew mates.

I had no problem with him learning to be a medic.
One class in biochemistry doesn't make you first aid qualified. I have a college degree in healthcare but that doesn't make me first aid qualified.
It makes no sense.
Janeway didn't volunteer him to be a medic, she volunteered him to train. Presumably he was good enough to pass his training.

I'm sure he learned first aid when he took it. We really don't know what Paris learned in his classes at the academy. It's the 25th century. Ten year olds know calculus in the 24th century. So who knows just how much training Paris has had in first aid.

They needed someone for the job, Kes and Neelix had already volunteered for stuff, and Janeway only needed the flimsiest excuse to give him an opportunity to be more useful and less cocky.

He went through starfleet academy. He learned first aid. You live in the primitive 21st century.
My larger point is that in order to give pilots development it has, historically in the franchise, been off the bridge and outside a pilot's skill set.
If Ortegas' needs development it will likely occur away from the bridge and complaints will surface about her being the wrong person for that job.

Star Trek has always struggled with the helms-people, often to the point that I wonder why they are often included in the main casts, and even TNG learned the pilot didn't need to be main cast after Wesley left.
TNG learned the lesson and reduced it to guest stars and seat warmers. Then forgot the lesson.
