Four come to mind and that one's a toughie. As much as I'd love to guess that that is part of an inner nose, my first guess would be "Terror of the Zygons".

(Is there a time interval between guesses? I'll chill out for a few hours, cleaning the ice box or watching videos of penguins ice skating or something...)

Zoiks! That story was the 4th on my list to have mentioned and Axos was already stated. I was really hoping for the Genesis clam as I could have sworn the cave set had spiky things in the background too, but I've not seen any of these in ages.
