Different departments have different needs. Have a medical one with a collar that secures across the neck would prevent exposure across in the event of fluids. There's also the possibility that Culber's is like that because he's the CMO, and gets a special variant. Sort of like how we saw McCoy wear his short sleeve uniform much more often on the Enterprise than the regular one. Or how Kirk has his wraparound variant, or Picard got his variant starting with season 5's "Darmok". Picard and Sisko also seem to get an extra vest underneath their uniform with the FIRST CONTACT/later DS9 uniforms.
