Int. Titan Ready Room. Jack and Picard are sitting at a table, finally able to have the discussion they need.

PICARD: So Jack, one thing is bothering me... Why do you look so fucking old?
JACK: "Mum told me there would be a trade off... something about my father's genes; and either I undurgo a procedure she designed that ended up with me looking like this, or else I would have been completely bald-headed at 18..."

I don’t know if I would have made Worf’s hair that white. Klingons have longer lifespans than humans and so he would be only in his late 40’s comparison wise
 
She can probably recreate the sperm if needed.
I doubt it. I also doubt that it would be legal, given Federation laws about genetic engineering and the need for consent to be obtained for such things. But, since Picard is the biological father, she clearly did not. Her dead husband is neither the legal nor the biological father.

Re: her explaining to do... wondering if the last time Picard saw her (which he references in Ep01 and said it wasn't left on the best of terms) will come up again and explain it. Maybe she asked him to quit Starfleet and move to the suburbs and start a family with her and he said no, I'm over that. I like Jack a lot but I can't fathom how they're going to sell this.

Also, I've now read two reviews where the reviewers think that Jack didn't know Picard was his father until Picard said it over comms. That wasn't the impression I got at ALL- Jack clearly knew a lot about Picard (comments about his rank, his vineyard) , his sincere "well it was nice meeting you" when he thought they were going to die, the uncomfortableness with the "who's your father" conversation. Am I seeing something that's not there? My assumption is that Bev told Jack everything and Jack knew exactly who Picard was.

Reading the subtext, I assume he does. I also assume from the look on his face that Picard had a pretty good inkling by the time he demanded “Who was your father?” and Jack knew that he knew and resented it when he fired back “I never had one!” Looking back at “Tapestry,” Jack actually is strikingly like the young Picard in personality and even looks. Circumstances have also made him as loyal to his mom as Picard was to his. I’m not sure what Beverly told Jack to keep him from wanting to look Picard up himself.

Humans prematurely bald and/or turn white. Why not any others like Klingons or Romulans?

I agree that is it odd Worf would be that gray so young, considering all the older, middle-aged, and young Klingons we have seen.

However, given Worf's Starfleet career, it's entirely possible he went through a situation like Teal'c did at the end the STARGATE SG-1 finale... a time dilation field where he aged decades in a few seconds. (Really, decades passed when the Odyssey was in a subspace field that stopped time, and by the time Carter figured out how to get back, someone had to go back and change things and only Teal'c, with his lifespan of almost double that of humans, was the logical choice. He kept his gray streak of hair, though.)

Or some other weird scenario. Remember the mantra...
