I think it's clear that the real villains of this episode are Harry Kim and Captain Janeway. If Harry hadn't of illegally used time travel neither 7 or Icheb would have even been in Federation space to ending up suffering like they have. And if Janeway hadn't subsequently illegally used time travel to get the Voyager home early, they likely would have never have been put in the situations the were put in.
Yeah, they would have both died in the Delta Quadrant during a massive intergalactic war with Species 8472.
Compared to how I remember the past, it seems in the 10s our society developed an addiction to outrage, with indignation become part of life.
It has always been there, it's just amplified by the Internet and encouraged by our current Leader here in the USA.
If they suddenly beam out all the mechanical parts their victims would die, too. They are after all very integrated in the body. It would be faster and more practical, too. If the goal is just to get the parts out of the ex Borgs and the persons doing it would just be cold and unemotional and just doing their job, they would choose this course. Kind of like a butcher killing an animal. Killing it would not be something enjoyable, but also not something affecting him emotional in a negative way. It is just something to be done to get meat. So that this woman instead used the slow method and extracted one part of Icheb after another and also kept talking to him implys to me that she enjoyed torturing her victim for an extended time. I think just beaming all mechanical parts out of him would have killed him way faster and she also would have finished her job faster.
Heh ... I'm thinking that that particular lady wasn't a Surgeon.
Seemed more like a 'chop-shop' technician.
Thus the hiring of Mr. Vup.
Henchman and Body-Cleanup-Guy all in one.
Not only that, but he works for bare-bones as well.
Beaming Borg implants out doesn't seem to have come up before. In BOBW, Dr. Crusher talks about removing the implants as a matter of surgery, not pushing a button on the transporter.
As for why beaming them out won't work... who cares? Any answer would just be technobabble anyway. Wouldn't be the first time that transporters can't make a problem just go away even though in theory they ought to be able to.

She said micro surgery, which may well have involved transporter tech. Her hands aren't that small
Doesn't matter in Picard. These folks are working in a chop shop scrounging for valuable parts, Crusher was an ethical doctor looking to save lives.
Assuming these guys work like chop shops for cars, they don't work in one place. They'd get bodies in fast, rip out the parts faster and go to a new location for the next delivery so they aren't found. They would want cheap easily moved, used, and maintained equipment not something that needs skill and maintenance just to keep in operation like a transporter when a knife and a power saw will do.

It takes a lot to shock me, but what happened to Icheb is traumatising. I guess it because the last time we saw him he was just a boy with a Kirk fixation and dreams of becoming something more. What makes it worse that the majority of Icheb's life is a tragedy but he'd found happiness on Voyager and later made his dream of becoming a starfleet officer come true. For Icheb to die under such horrific circumstances is just wrong. I'm usually not affected by the deaths of fictional characters, but this one is really haunting me.
I wonder if the reason Seven seems to be isolated from the Voyager crew is because she blames them, specifically Janeway in some way. I wonder if Seven took out some of that rage on Janeway for tampering with time and bringing the crew home early. Maybe in the timeline Future Janeway wanted to avoid Icheb lived and Seven knew this. I doubt we'll get a call out to this but it would be nice to have a flashback to Janeway trying to comfort Seven but only receiving unreserved anger for the loss of her surrogate son.

The episode makes it quite clear Seven of Nine blames herself. As she herself said, if it weren't for her, Bjayzl wouldn't even know about Icheb. It's hard to see how any other Voyager crewmember could take the blame in this scenario.
I'm aware that Seven blames herself. I'm talking about misplaced rage and how it may have impacted her relationship with her former crew.
