My birth certificate says that I was born in Lancaster, Pennsylvania, USA. Presumably that location plus the date will be enough for anyone interested to look up the birth register to confirm it, for example if I ever run for president of the USA.
My assumption is that the United Earth is a bit more united than a more functional United Nations, and that countries are no longer sovereign, except in the limited sense that US states or Indian reservations are sovereign.
As I remember, most of the Roman Empire consisted of city states called civitates which included subdivision called pagi Civitates were part of provinces which were part of the Roman Empire. So during the early empire the hierarchy was pagus, civitas, provincia, imperium. That is vaguely similar to the municipality, county, state and federal governments in the USA.
In the later Roman Empire two more levels were added. A group of provinces became a diocese ruled by a vicar, and a group of dioceses became a prefecture ruled by a praetorian prefect under the emperor.
So during the later Roman empire there were six levels of government.
So Ben Sisko when he lived in New Orleans was a resident of the municipality of New Orleans, (and possibly of a parish which is a county equivalent), the state of Louisiana, the United States of America, the United Earth, and the United Federation of Planets.
And I suppose that the United Federation of Planets is the government level which has sovereignty.
There is nothing wrong with conjecturing which continent, country, state, county, or municipality Amanda was born in.

There is however 48 states and Florida on the continent of America, who have a fair working relationship.

The USA still exists, even in Trek's time. It's simply part of a greater whole.
USA is a member nation of United Earth, which is a member world of the Federation.
My suggestion is that although the individual states still exist, just that they are united by the united Earth constitution, not the united states constitution.

The old nation-states probably exist within United Earth the same way that Federation member worlds exist within the UFP.
Meaning: They are left to run their own local affairs as they see fit, provided that they abide by basic UE laws.

He'd still be the political leader of a region that includes a billion or so people and responsible for ensuring their welfare. Not sure if it matters if he's a mid level government official instead of a top level one
Then there's the question of elections.
If the President is appointed from above rather than elected by the people, how much power should have be given?

I see no reason why he wouldn't be elected.
He'll have power over issues that affect the people in the region he governs.
It's in Polynesia right now. Why would that change? So's New Zealand.
