In universe the whole of Starfleet is run by Americans, its NASA in space. Its the only way they could remain a superpower once Earth was united.
My assumption is that "United Earth" is just a future version of the United Nations that actually works, and that countries are still sovereign.

My suggestion is that although the individual states still exist, just that they are united by the united Earth constitution, not the united states constitution.
The old nation-states probably exist within United Earth the same way that Federation member worlds exist within the UFP.
Meaning: They are left to run their own local affairs as they see fit, provided that they abide by basic UE laws.
