I just watched the episode before I selected it. The transcripts don't do the refusal scene justice. McCoy was dead set against treating the Horta, refusing three times. Kirk's order to treat it was forcefully delivered (Shatner style):

KIRK: It's wounded. Badly. You've got to help it.
MCCOY: Help that? (Refusal #1)
KIRK: Go take a look.
(McCoy kneels by the wound and gets out his tricorder.)
SPOCK: The end of life. Murderers.
MCCOY: You can't be serious. That thing is virtually made out of stone! (Refusal #2)
KIRK: Help it. Treat it.
MCCOY: I'm a doctor, not a bricklayer. (Refusal #3)
KIRK: You're a healer. There's a patient. That's an order.​
