I just watched the episode before I selected it. The transcripts don't do the refusal scene justice. McCoy was dead set against treating the Horta, refusing three times. Kirk's order to treat it was forcefully delivered (Shatner style):"I'm a doctor, not a bricklayer" is a refusal (and a hilarious one at that). Kirk orders him to treat Mother Horta anyway.
