Obviously, the most gifted cadets can choose a more rigorous curriculum that results in graduation as an Lieutenant rather than an ensign. I would assume that immediately after graduation you can also go to command school where you would take the Kobiashi Maru and teach classes to underclassmen. But you would still be referred to as a cadet during that time.

If the ship is part of some secretive, elite organization that operates outside regulations, then maybe they can develop that and Tilly's character. The same goes for Lorca, who at times strangely gives in to Stamets' taunts. I'd like to see more scheming and backstabbing together with greater brutality towards foes.

Kirk also met Gary Mitchell while at the Academy and their first encounter was 15 years prior to the second TOS pilot, and that meeting was when Gary was attending a class taught by student instructor Kirk.
